The device, used in treatment, was returned for evaluation.A visual inspection of the returned drill confirms the tip of the device broke off.The broken piece was not returned with the device. The device was manufactured in 2013.The device shows signs of significant wear and use.A medical investigation was conducted and confirms without the requested clinical information, operative report, radiographs or the device, the root cause of the tip breakage cannot be determined.Although, we cannot rule out a user error as the likely cause of the reported event. The retained approx.3cm tip of the 6.4 mm drill bit is a non-implantable device comprised of 17.4 ss, that is not intended for long term exposure with skin or tissue.Since it was reported the drill bit tip was retained in the neck of the patient¿s femur, the possibility of local skin irritation, micro-motion/migration of the retained tip is unlikely. Therefore, the impact to the patient was the retained non-implantable drill bit tip and the with greater than 30-minute surgical delay.Since it was reported there were no patient injuries, no further clinical/medical assessment is warranted at this time.Should additional relevant clinical information be provided, the clinical/medical will be re-assessed a review of the complaint history on the listed part revealed no prior complaints for the listed failure mode with the same batch number.A review of the manufacturing records did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of the risk management file revealed this failure mode was previously identified. This device is a reusable instrument that can be exposed to numerous surgeries; damage from repeated use can occur.Expected wear/tear is likely the probable cause of the reported event. We recommend that all reusable instruments be routinely inspected for wear and damage and replaced as necessary. At this time, we have no reason to suspect that the product failed to meet any product specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaints will be reopened. No further investigation is warranted for these complaints; however we will continue to monitor for future complaints and investigate as necessary.(b)(4).
