> pc-(b)(4).Visual inspection: the inspection of the sleeve has shown that the first thread flank is deformed, no breakage could be observed.Otherwise is the device in a good condition with just slight wear marks. The complaint is rated as confirmed for the sleeve as the first thread flank is deformed.During the performed evaluation no manufacturing related issue could be detected.Based on the provided information the exact cause cannot be defined, the description states that the issue was detected preoperative. However, the damage itself is consistent with the damage of the also received screw with the cracked thread on top as it looks like that the screw and the sleeve did get damaged during the same procedure by applying too much lateral stress onto the devices.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed as no product related issue could be detected.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post market safety surveillance activities.Device history lot
