Event or Problem Description
It was reported that balloon rupture occurred. The 70-80% stenosed target lesion was located in the non-tortuous and moderately calcified arteriovenous fistula.A 8.0 x 100, 75cm mustang balloon catheter was advanced for dilatation. However, during inflation at 20 atmospheres (atm), the balloon ruptured.Subsequently, another 8.0 x 100, 75cm mustang balloon catheter was advanced for dilatation, but the balloon also ruptured at 20 atm. Both devices were removed and the procedure was completed with another of the same device. No patient complications were reported and the patient's status was stable.

Additional Manufacturer Narrative
A 8x10x75cm mustang device was received for analysis. An examination of the returned device identified that the balloon had been inflated. The returned device was attached to an encore inflation unit and positive pressure was applied in an attempt to inflate the balloon. The balloon could not be inflated due to the presence of a shaft leak.A microscopic examination of the balloon material identified no issues which could potentially have contributed to this complaint. No issue was observed with the tip or markerbands of the device which could have contributed to the complaint incident.A visual and tactile examination found the shaft of the device to be severely stretched/damaged at more than one location. This type of damage is consistent with excessive tensile force being applied to the device.During analysis the returned device was attached to an encore inflation unit and positive pressure was applied in an attempt to inflate the balloon of the device and liquid was observed exiting from the site of the severe shaft damage located approximately 5mm proximal of the proximal balloon bond.No other issues were identified during the product analysis.
