This report is being submitted as follow up no.1 to update section h3, and to provide the completed investigation results. One 6fr 45 cm destination sheath and dilator were received for product evaluation.Visual inspection of the dilator and sheath revealed damage at the distal tip of the sheath; no damage was seen on the dilator.Microscopy confirmed that the sheath tip was damaged in a fashion where there were folds on the distal edge of the sheath along with some jagged edges and a bulge and a small tear in the shape of a v-notch were exhibited. The outer diameter of the sheath was also found to be in manufacturer specification. The inner diameter of the dilator was measured to be in manufacturer specification. The complaint can be confirmed for sheath tip mechanical damage.Based on the damage observed, it is likely that the damage on the sheath tip can be attributed to difficulties at the point of insertion, possible due to scar tissue or calcification. The device was in a conforming state when released from terumo control. There is no indication that any manufacturing issues may have led to this event.Currently no action is recommended since this risk evaluation is within the predetermined limits in the fmea.
