Event Description
Medtronic received a report that the imaging after a pipeline procedure showed that aneurysm neck coverage was not achieved.Wall apposition was achieved, and the raymond <(>&<)> roy classification was class 3.The patient did not experience any injury or complications.The devices were prepared according to the instructions for use (ifu).The patient was undergoing treatment for an unruptured, saccular aneurysm located in the right posterior communicating artery at the c7 segment.The max diameter was 5.9mm, and the neck diameter was 5.7mm.The landing zone was 5mm distal and 4.9mm proximal.Dual antiplatelet treatment was administered, and the pru level 86.Patient medical history included hypertension, sah, and hypoacusia.Ancillary devices include a phenom 27 microcatheter.

Additional information received reported corelab determined wall apposition was not achieved.The site note in response: "in the proximal length of the stent there is [not] a good wall apposition because the irregularity of the artery wall.".

Event Description
Additional information received reported that corelab imaging showed that wall apposition at the full length of the stent was not achieved at the 1-year follow-up imaging on (b)(6) 2021.The patient's aneurysm occlusion status was raymond and roy class 3 at their 180-day follow-up imaging on (b)(6) 2020 and at their 1-year follow-up imaging.The aneurysm occlusion was either better or at least equal to their previous imaging.The site confirmed the aneurysm occlusion status.This was not due to a device deficiency, and no additional medical intervention/treatment was needed at this time.
