The associated device, intended for use in treatment, was returned and evaluated.A visual inspection of the returned device confirmed the stated failure mode. The device was found to be stripped and worn, rendering the device inoperable. The device shows signs of extensive use.A complaint history review found related failures; this failure mode will be monitored for future complaints and assessed for any necessary corrective actions. At this time, we do not have reason to suspect that the product failed to meet any product specifications at the time of manufacture. This device is a reusable instrument that can be exposed to numerous surgeries.Damage from prolonged use, misuse or rough handling are likely probable causes of the reported event. We recommend that all reusable instruments be routinely inspected for wear and damage and replaced as necessary.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary.

Internal complaint reference: (b)(4).Further review by the manufacturer has identified that this event should be re-evaluated for mdr reporting. The event states that the flexible screwdriver was found damaged during inspection without patient involvement.Users are instructed to inspect all instruments before use and not to use the device if any damage is found.Device damage that is identified during the inspection process will not cause or contribute to a serious injury or death, nor would it be likely to do so in the event of recurrence as the inspection process itself mitigates the risk of any potential harm occurring. Therefore, it was determined that this case does not meet the threshold for reporting and is a non-reportable event. If additional details confirm the occurrence of a reportable event, we will update our records accordingly and submit a subsequent report detailing both the event and our complete investigations. While this event is no longer considered reportable, this report is being submitted as a courtesy to provide updated investigation results based on the return and evaluation of the complaint device subsequent to the submission of our previous supplemental report. The associated device, intended for use in treatment, was returned and evaluated.A visual inspection of the returned device confirmed the stated failure mode. The device was found to be stripped and worn, rendering the device inoperable. The device shows signs of extensive use.Device batch number was not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history of the previous 12 months revealed similar events for the listed part number, this failure mode will be monitored for future complaints for any necessary corrective actions. This is a single use device, surgical technique or damage from misuse are likely potential factors that could contribute to the reported event. The device should not be reprocessed or reused if comes in contact with blood, tissue or bodily fluids.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
