Additional Manufacturer Narrative

|
This report has been identified as b.Braun medical internal report number 400500489.One photo and one used sample without packaging was provided for evaluation.Visual evaluation of the photo showed the lidstock confirming the reported lot number. It did not confirm the defect.Visual evaluation of the sample showed two separate pieces of the pencan needle g25 x 3 1/2" that had been bent and broken. The approximately one (1) inch tip portion appeared bent, damaged and excessively manipulated.Although the needle was returned broken and damaged, it does not appear to be related to any manufacturing or packaging process. The exact root cause was not able to be determined. However, since the needle had been used and multiple attempts of insertion were reported, the damage and breakage does not appear to be the result of any manufacturing or packaging process.Review of the discrepancy management system (dsms) database was performed for the reported lot number and no abnormalities or non-conformances were noted during the in process or final product inspection. We will maintain this report for further references and continue to monitor other reports for similar occurrences. If any additional pertinent information becomes available, a follow up will be submitted.
