The device has not been returned to omsc for evaluation.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
A customer reported that multiple indicators on the front panel sometimes flashed and became unusable.This event was found during the preparation for use.Then, olympus inspected the device at olympus service operation repair center (sorc) and found that ac power inlet was burnt.And brightness was not adjusted due to diaphragm failure.The inspection of the device also found the followings; -the light guide connector was worn.-the endoscopic image was dark due to the weak light.-the device did not work properly due to the defects of the turret and mesh turret.There was no report of patient injury associated with this event.

This supplemental report is being submitted to provide additional information.The subject device was not returned to olympus medical systems corp.(omsc) for evaluation.Omsc reviewed the manufacture history (dhr) of the subject device and confirmed no irregularity.The exact cause of the reported phenomenon could not be conclusively determined.However, based upon the information from olympus service operation repair center (sorc), blinking of the multiple indicators on the front panel could have been attributed to aging deterioration of the aperture blades, the mesh turret and the filter turret due to repeatedly long-term use.The ac power inlet burnt could have occurred due to repeatedly long-term use.
