The device has not been returned to omsc for evaluation.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
A customer reported that the device couldn't turn on during a procedure.Reportedly, when the customer turned off the device and tried to turn it back on, the device did not turn on.Then, olympus inspected the device at the service department of olympus (b)(4) and found power supply and aperture failure.No endoscopic image was displayed on the monitor.There was no report of patient injury associated with this event.

This supplemental report is being submitted to provide additional information.Other than the events reported in b5 of the initial mdr, there were no defects confirmed by device inspection by olympus korea co., ltd.Device history record review indicates that the device was manufactured and tested in accordance with all applicable procedures and met all final product release criteria.The exact cause of the reported event could not be conclusively determined.However, olympus medical systems corp.(omsc) assumed that the reported event was caused by the defect of the power supply unit.This defect may have been caused by aged deterioration due to long-term use.If significant additional information is received, this report will be supplemented.
