Event or Problem Description
It was reported that the surgeon was unable to get the replacement insert to properly engage after several attempts (catalog # 122072132, lot # 8506054).He then selected a 32 +3 10 degree l series inserts.After several attempts he was able to get it implanted and the 3 replacement screws were placed.I was unable to retrieve the original replacement insert.Doe: (b)(6) 2021.Right hip.
