Device was evaluated.Evaluation found that the device has no motor function. The device when tested failed phase a, b and c to gnd. There were no other abnormalities noted in the inspection. The original equipment manufacturer (oem) performed a device history record review and no abnormalities were noted. An investigation was completed by the oem and determined that there is no manufacturing nor processing related cause for the reported failure mode. The root cause has been determined to be material and assembly of the cable wires inside of the mdu.Hall sensor signal wires and motor phase wires in the cable become compressed with braided shield resulting in insulation damage and shorting of the conductors to ground. These occurrences will cause the handpiece to not function as intended with shortages and display error messages.Olympus will continue to monitor the field performance of this device.
