H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The following were reviewed as part of this investigation: patient severity, trend analysis, applicable previous investigation(s), sample (if available), and applicable fmea documents.Based on a review of this information, the following was concluded: the complaint of a broken stylet was confirmed; however, the root cause was not identified. The product returned for evaluation was one 5fr d/l powerpicc catheter.Usage residues were observed throughout the sample.A fragment of tls stylet was received with the catheter. The fragment included the distal tip and several magnets.Kinks were observed between several of the magnets.Microscopic inspection of the stylet revealed discoloration and deformation in the vicinity of the break. The tubing exhibited an irregular and elliptical cross-section at the break site. The polyimide tubing wall thickness was measured at multiple points using a digital microscope. The measurements were found to be within manufacturing specifications.Manipulation of the stylet appeared to contribute to the observed device deformation; however, an additional unidentified factor(s) may have contributed.Consequently this complaint is confirmed as ¿cause unknown¿ at this time.
