Event or Problem Description
During a procedure an attempt was made to use one resolute onyx rx coronary drug eluting stent to treat a lesion located in the distal circumflex (cx) artery. The device was inspected with no issues noted.Negative prep was not performed. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was encountered when advancing the device.Excessive force was not used during delivery. It was reported that stent dislodgement occurred during removal following a failed delivery. It was stated that problems crossing the lesion in the distal left cx occurred and a decision to switch from a radial to femoral approach was made. When removing the system from the patient's radial artery, the stent became dislodged when it was being pulled through the sheath. The stent was massaged out of the access site by hand and was successfully removed from the access site.A 2.25x22 resoluteonyx was successfully delivered via femoral approach. The patient was reported to be alive with no further injury.
