Event Description
It is alleged that the patient received a gunther tulip. It is alleged that the patient was injured without further explanation.Hospital and medical records have been requested, but not yet provided.

Event Description
Patient allegedly received a cook gunther tulip filter (lot 2422975) on (b)(6) 2010 via the right internal jugular vein prophylactically prior to bariatric surgery. This filter was successfully removed (b)(6) 2010.No allegations have been made against this device. The patient alleges subsequently receiving a cook gunther tulip filter (lot e2717587) on (b)(6) 2011 via the right femoral vein due to status post pulmonary embolism (pe). This filter is the subject of this report.Patient is alleging device migration and vena cava perforation.Patient notes and further alleges experiencing "anxiety".Per a ct(2) abdomen without contrast: "filter type: cook gunther tulip.Ivc stenosis: none.Filter migration: none.Filter tilt: none"."impressions: some of the filter struts have penetrated 3 mm through the wall of the ivc into the pericaval/mesenteric fat.Half of the filter is above the renal veins. It may have been placed high or may have migrated to this position.A high position of the filter puts the patient at risk for renal vein thrombosis and renal failure if the ivc becomes thromboses".Per a ct abdomen without contrast: "no enlarged retroperitoneal lymphadenopathy. The superior aspect of the ivc filter is just above the origin of the renal veins.Near l1 and l2 vertebrae".
