H4 manufacturing date ¿ added h3 device evaluated by mfg ¿updated h3 summary attached - updated d4 expiration date - added d9 product available to stryker ¿ updated d9 returned to manufacturer on ¿updated the device was returned and the lot number was confirmed with the packaging returned with the device. The main coil was not returned.During visual inspection, the delivery wire was noted to be kinked bent and was detached/separated.Functional test unable to be preformed due to no coil been returned.Due to the automated mes system there are controls in the manufacturing process to ensure the product met specifications upon release. The reported event is covered in the device directions for use (dfu). As well, the risk of the reported event is documented in the risk documentation and there are current controls to mitigate the risk of the as reported event. The reported complaint was confirmed based on analysis. The device failed to meet specifications when received for complaint investigation based on the analyzed anomalies noted to the device. As per the available information, there were no anomalies noted to the device prior to use, the device was prepared as per the dfu, continuous flush was maintained for the duration of the procedure, the coil couldn't be advanced in the micro catheter distal part.(at the acute angle) and the micro catheter was located at the acute angle ica to a1.It is probable that the damage to the device occurred during the attempt to advance beyond the acute angle ica to a1.An assignable cause of procedural factors will be assigned to the reported main coil prematurely detached/separated during use, coil in catheter friction and to the analyzed coil delivery wire kinked bent and main coil prematurely detached/separated during use, as this complaint appears to be associated with a product that met stryker design and manufacturing specifications and was used in accordance with the dfu, but performance was limited due to procedural factors during use.
