Original.It is alleged that the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2016.It is alleged that the patient was injured without further explanation.Hospital and medical records have been requested, but not yet provided.

Event Description
Patient allegedly received an implant on (b)(6) 2016 due to deep vein thrombosis.Patient is alleging tit, vena cava perforation.Patient further alleges fear.Report from computerized tomography (ct): "there is an infrarenal ivc filter with tip immediately inferior to the renal vein insertions.The tip is along the posterior wall of the ivc indicating some tilt.The 4 longus tines extend beyond the lumen of the expected confines of the ivc and the pericaval fat." "ivc filter as above.".
 
Event Description
Per a report from computed tomography 2; ¿the cone of the filter is lying against the left posterior ivc wall.Left filter strut penetrates 12 mm through the ivc wall.Right strut penetrates 9 mm through the ivc wall.Anterior strut penetrates 7 mm through the ivc wall.Posterior strut penetrates 15 mm through the ivc wall.¿.
