One resolute onyx coronary drug eluting stent was used during a procedure to treat a severely tortuous, severely calcified lesion with 90% stenosis in the proximal circumflex (cx) artery. The device was inspected prior to use with no issues noted.Negative prep was performed with no issues noted. The lesion was pre-dilated with multiple balloons. The device passed through a previously deployed stent.Resistance was encountered when advancing the device and excessive force was used during delivery. It was reported that stent dislodgement occurred during delivery to the lesion. It was stated that the stent got stuck in the first stent and was wedged. The stent was then deployed with a smaller 1.25mm balloon and worked up to a 2.5x12mm balloon. No further patient injury reported.
