Additional Manufacturer Narrative
Pma/510(k) #: k163018.Device evaluation: the zilbs-635-10-6 device of lot number c1690208 involved in this complaint was returned for evaluation, with the original outer packaging. The packaging was open on receipt. With the information provided, a physical examination and document-based investigation was conducted.Lab evaluation: the device related to this occurrence underwent a laboratory evaluation on the 23 april 2021.On evaluation of the device a kink was observed approximately 38.0 cm along the outer sheath from the distal end of the handle.A second kink was observed approximately 110.0cm from the distal end of the handle along the outer sheath. The device flushed and a 0.035¿ wireguide passed as expected. The wireguide passed the kinks on the outer sheath with no issues.A black wire was returned in the delivery system measuring 0.029¿.The red safety lock was in place on return. It was removed during the lab evaluation. An attempt to deploy the stent during the lab evaluation was unsuccessful. There was no defect observed on the outer tip.Following the lab evaluation images were provided to the customer and additional information was requested.Earlier customer feedback referred to ¿the white plastic part¿ of the device which they later referred to as the ¿shelf-less tip¿.The lab evaluation report refers to this part of the device as the distal white tip.Earlier feedback also referred to ¿separation¿.Additional clarification was sought from the customer who confirmed ¿separation¿ as ¿the shelf-less tip starts to separate from catheter before deployment¿.However, no separation was observed during the lab evaluation. The customer also confirmed that the black wire returned with the device was the wire guide used in the procedure.Document review: prior to distribution zilbs-635-10-6 devices are subjected to a visual inspection and functional checks to ensure device integrity. These inspections and functional checks are outlined in internal procedures in place at cirl.A review of the manufacturing records for zilbs-635-10-6 of lot number c1690208 did not reveal any discrepancies that could have contributed to this complaint issue. The review of relevant manufacturing records confirms the failure mode has not previously occurred with the current lot number.Based on the information available to date there is no evidence to suggest that there are any manufacturing issues associated with lot number c1690208.It should be noted that the instructions for use (ifu0065-3) states the following: delivery system the delivery system accepts a.035 inch wireguide there is evidence to suggest the user did not follow the instructions for use.The customer confirmed that the black wire returned with the device was the wire used for the procedure. When this wire was measured during the lab evaluation the noted measurement was 0.029 inch.Root cause review: a definitive root cause of user error was identified from the available information as an incorrect wire guide size was used. The stent could not be deployed due to kinking.Resistance was also evident when attempting to get the device into position. These issues may have been because of using the smaller than recommended wire guide as this may provide insufficient device support.Summary: complaint is confirmed as the failure was verified in the laboratory.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.Complaints of this nature will continue to be monitored for potential emerging trends.
