Investigation ¿ evaluation. It was reported by, (b)(6), ¿the tip of the catheter broke off in the patient.¿ as reported, on (b)(6) 2021, an unknown patient required the use of a liver access and biopsy set (rpn: labs-100-j, product lot number: 10333585) during an unknown procedure. The operator reported that initial access was gained via the right hepatic vein using the amplatz curved wire guide and straight catheter that is included within the set.During removal of the catheter, the catheter separated, and the tip remained in the patient¿s right hepatic vein. The separated piece was removed using a snare during the same procedure. The procedure was completed using another similar device (unknown rpn, unknown product lot number). No adverse effects to the patient were reported.Reviews of the complaint history, device history record (dhr), quality control, manufacturing instructions (mi), and instructions for use (ifu), were conducted during the investigation. The complaint device was not returned; therefore, no physical examinations could be performed. However, a document-based investigation evaluation was performed.A review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution.A review of the dhr for the reported complaint device lot revealed no recorded non-conformances relevant to the failure mode.A review of the sub-assembly lot found a related nonconformance, the nonconforming material was scraped.A database search did not identify any other events associated with the reported device lot.Based on the available information, cook has concluded that there is no evidence suggesting nonconforming product exists either in house or in field.Cook also reviewed product labeling. The ifu packaged with the device contains the following in relation to the reported failure mode: warnings: ¿extreme care must be exercised during manipulation and withdrawal of catheter to prevent pulling catheter apart.Insertion through a synthetic vascular graft should be avoided whenever possible.¿ instructions for use: ¿-note: care should be taken to prevent damage to the straight catheter during removal through the metal stiffening cannula.Leaving a wire guide through the straight catheter during removal may aid in preventing catheter damage.¿ how supplied: ¿-upon removal from package, inspect the product to ensure no damage has occurred.¿ based on the information provided, no product returned, and the results of the investigation, the cause of this event is component failure. It is possible that inadvertent user error contributed to the catheter tip to break, though this cannot be confirmed without additional information. The appropriate personnel have been notified.Cook will continue to monitor for similar complaints.Per the risk assessment no further action is required. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
