Event Description
It was reported that stent damage occurred.A 3.00 x 12 synergy ii drug eluting stent was selected for use; however, it was noted that there was noticeable damage to the corner of the box.There were no visible hole or tear on the inner package.Furthermore, on opening the package, the physician felt that the stent had been compromised since the stent metal was bent and decided to discard it.The procedure was completed with another of same device.There were no patient complications reported.
