Block h6: medical device code a0414 captures the reportable event of stent torn inside the patient.Medical device code a040601 captures the reportable event of stent buckled inside the patient.Block h10: the returned polaris ultra ureteral stent was analyzed, and a visual evaluation noted that the proximal (bladder pigtail and suture hole) section was torn. The suture string was not returned with the device. No other problems with the device were noted. The reported event was confirmed. The analysis of the returned device revealed that the bladder pigtail and suture hole of the ureteral stent was torn. The failure found (suture hole torn) is a problem that could have been generated by the user or due to the interaction of the device with the suture string.Additionally, the suture string was not returned and the device was outside the original pouch.During the product analysis, no buckled defects or issues were identify.Review and analysis of all available information indicated that the root cause is adverse event related to procedure.A complaint with a cause of adverse event related to procedure indicates that the adverse event occurred during the procedure and the device had no influence on the event.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.
