|
It was reported that during arthroscopy procedure, inside the patient, the light source 500xl xenon went black and the box was hot.No surgical delay.Unknown how the procedure was finished.Patient injuries were not reported.All available information has been disclosed.If additional information should become available, a supplemental report will be submitted accordingly.

The reported device, used in treatment, was received for evaluation.There was a relationship found between the returned device and the reported incident.A visual inspection was performed on the exterior of product and no physical damage was observed.A functional evaluation confirmed the complaint of power up failure.Cause of power failure is a defective power supply/ballast.Unit powered up and passed functional testing with a known good power supply installed.The complaint investigation has concluded the cause of the failure to be a defective electronic component.A complaint history review concluded this was a repeat issue.The complaint of overheating could not be confirmed due to the inability to power up the device in as received condition.Overheating was not observed when the unit was tested with a known good power supply.A review of the device history records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.No containment or corrective actions are recommended at this time.The complaint was confirmed.Factors, which could have contributed to the complaint event, include an application of unintended inappropriate or excessive force to the device.No containment or corrective actions are recommended at this time.
