The device has not been returned to omsc but was returned to ocsm for evaluation.Ocsm inspected the device and confirmed the following; the reported phenomenon was reproduced.The device did not start up due to the power supply board failure.The device has not been repaired in the past year.The exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed by the user that during the hysteroscopy, the device was not turned on.The user completed the procedure with the device.This device was used with a standard set for inspection.There was no report of patient injury associated with the event.Olympus inspected the device at the service department of olympus china sales & marketing (ocsm) and found that the lamp socket was broken.
 
Manufacturer Narrative
This supplemental report is being submitted to provide additional information.The exact cause of the reported event could not be conclusively determined, because the device has not been returned to olympus medical systems corp.(omsc).However, the reported power supply failure may have occurred due to deterioration of electronic components on the main board, because more than 10 years have passed since the device was manufactured and delivered.In addition, the lamp socket may have failed due to aging.Device history record (dhr) review indicates that the product was manufactured and tested in accordance with all applicable procedures and met all final product release criteria.If additional information becomes available, this report will be supplemented.
