Event or Problem Description
Medtronic received a report that the axium prime coil prematurely detached. The patient was undergoing treatment for a ruptured aneurysm located in the anterior communicating artery. The patient's blood flow was normal. It was reported that early detachment occurred when pushing and pulling were performed during implant in the aneurysm. The coil was implanted in the aneurysm using a guidewire and a balloon to assist. There had been no resistance during delivery, though the coil had been relocated. The delivery pusher was not rotated inside the patient's body.A continuous flush was used. the patient did not experience any injury. The devices were prepared according to the instructions for use (ifu).Ancillary devices include an sl-10 microcatheter.

Event or Problem Description
Additional information received from a healthcare provider (hcp), via a manufacturing representative (rep), indicated indicated the coil was not implanted in the intended location: the look deviated from the mother blood vessel. There was no surgical intervention, but the blood flow was poor and medication was given to treat. The vessel tortuosity was not strong, and it was difficult to investigate the cause of the issue experienced.
