H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The following were reviewed as part of this investigation: patient severity, applicable previous investigation(s), sample analysis, applicable fmea documents, applicable manufacturing records, and labeling.Based on a review of this information, the following was concluded: the complaint of a catheter leak is confirmed but the exact cause could not be determined from the video provided. One video sample of a 5 fr perqcath catheter was provided for evaluation. The catheter is shown outside of patient use; however, blood residue appears to be present on the surface.A syringe is attached to the hub and a clear fluid is being infused.Fluid is observed to leak at the region between the hub and molded wing. The damage present on the device could not be clearly inspected from the video and due to the lack of a returned physical sample. The event description indicates that resistance was experienced during stylet removal which may be a likely contributing factor to the cause of the catheter leak.Since fluid was observed to leak, the complaint is confirmed but the exact cause could not be determined. The instructions for use (ifu) caution, ¿never use force to remove the stylet.Resistance can damage the catheter. If resistance or bunching of the catheter is observed, stop stylet withdrawal and allow the catheter to return to normal shape.Withdraw both the catheter and stylet together approximately 2 cm and reattempt stylet removal.Repeat this procedure until the stylet is easily removed.Once the stylet is out, advance the catheter into the desired position.¿.
