An event regarding disassociation and wear/metallosis involving an accolade stem was reported. The event was confirmed.Method & results: -product evaluation and results: the device is returned in a used condition.Damage is consistent with the loss of taper lock was observed on the head and stem of the returned devices. No further material analyses were performed.-clinician review: a review of the provided medical information by a clinical consultant indicated: undated, unlabelled x-ray - ap right hip - uncemented tha with stem trunnion disassociated, dislocated and eroded from modular head which remains in acetabular component.Radiodense debris noted around hip joint. No clinical or pmh, no patient demographics, no confirmation of revision surgery or operative reports, no examination of explanted components. While the single x-ray appears to confirm the event description, insufficient data is presented to create a medical report for this case.-product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.-complaint history review: there have been no other similar events for the lot referenced. Conclusions: it was reported that the head dissociated from the stem.It was also reported that it seems to be metallosis. The device is returned in a used condition.Damage is consistent with the loss of taper lock was observed on the head and stem of the returned devices. No further material analyses were performed.Clinician review of the provided x-ray indicated that uncemented tha with stem trunnion disassociated, dislocated and eroded from modular head which remains in acetabular component.Radiodense debris noted around hip joint. The exact cause of the event could not be determined because insufficient information was provided.Further information such as pathology reports, pre- and post-operative x-rays and the primary operative report as well as patient history and follow-up notes are needed to complete the investigation for determining root cause. No further investigation for this event is possible at this time.If additional information becomes available to indicate further evaluation is warranted, this record will be reopened.
