The device is expected to be returned for evaluation. It has not yet been received.A follow-up report will be submitted with all additional relevant information.

Event or Problem Description
It was reported that the procedure was to treat a mildly tortuous, moderately calcified left anterior descending artery. For pre-dilatation, a 3x12mm trek balloon was advanced to the lesion with some resistance with anatomy but ultimately reached the lesion.During the first inflation at 12 atmospheres, the balloon only partially inflated. When attempting to deflate, negative pressure was held around 5 seconds to deflate the balloon, but the device failed to completely deflate.Ultimately the device was simply withdrawn fully inflated.Another trek balloon was used to successfully complete the procedure. There were no adverse patient effects and no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
Visual, dimensional and functional inspections were performed on the returned device. The reported inflation issue and failure to deflate were not confirmed. The reported difficulty advancing the device could not be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints from this lot.The investigation determined the reported difficulty advancing the device appears to be related to operational context; however, the investigation was unable to determine a conclusive cause for the reported inflation issue and failure to deflate. There is no indication of a product quality issue with respects to the design, manufacture, or labeling of the device.
