Device evaluation: the suspect medical device was returned to the manufacturer for evaluation/investigation.The investigation confirmed that the immobile jaw part of the needle holder is broken off.The surfaces of the fracture show slight corrosion. The cause of this damage is most likely material fatigue caused by acute wear and tear.In view of the age of the product, it is assumed that the article, which, according to its product specification, is designed for 400 reprocessing and ultrasonic cleaning cycles, was used far beyond its expected service life.Furthermore, the remaining jaw part shows clear pressure marks outside the carbide plate and the pull rod is strongly deformed. This damage was most likely caused by improper handling. Thus, this event/incident was attributed to use error.A material or quality problem can be excluded since a manufacturing and quality control review was performed for the affected lot number of the needle holder without showing any abnormalities. The case will be closed on olympus side with no further actions. However, the reported event/incident will be recorded for trending and surveillance purposes and the user will be informed about the investigation results.
