Additional Manufacturer Narrative
Investigation summary: two photos of a 3ml syringe package confirmed to be from batch 0339480 (p/n 309574) were received and evaluated. Both photos showed the graphic side of the package.Further evaluation could not be performed because the syringe was not visible in the photos.Furthermore, a device history record review showed no rejected inspections or quality issues during the production of the provided lot number that could have contributed to the reported defect.Investigation conclusion: complaints received for this device and reported condition will continue to be tracked and trended.Information will be captured on trend reports and monitored monthly. Our business team regularly reviews the collected data for identification of emerging trends.Root cause description: during manufacture of this product torque removal testing is performed to ensure needles are properly attached and within specification. It is possible that packaged product is introduced to conditions that could affect the connection after leaving the plant.A physical sample would allow for testing to be performed to confirm if any manufacturing defects are present. The user should also verify a proper connection before using the device.Rationale: since no samples displaying the reported condition were received corrective actions are not necessary.

Event or Problem Description
