(b)(4).The returned cre wireguided balloon was analyzed, and a visual evaluation noted that no damages were found on the balloon and catheter of the device.A functional evaluation noted that the balloon was able to inflate but did not hold pressure due to a pinhole in the balloon. No other problems with the device were noted.Analysis of the returned device revealed a pinhole in the balloon material. The problem found could have been interpreted by the customer as the reported event of balloon leak.It is possible that interaction with scope or any other surface during the procedure could create friction on the balloon, causing the problem of "balloon pinhole" during the procedure. Therefore, the event is classified as "adverse event related to procedure" because, the adverse event occurred during the procedure and the device had no influence on event.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.
