Additional Manufacturer Narrative
Patient's age, date of birth unavailable.Patient's weight unavailable.Device was evaluated by manufacturer; the initial device evaluation is captured in the narrative below.(b)(4).Device evaluation: the device was returned to the manufacturer and was evaluated on 20 may 2021 by a cross functional team. The device itself was returned; however, the distal marker band was not returned.Slight erosion was noted on the distal tip of the device. There were two dead fibers present at the distal tip (within spec).A side view of the device was performed.Initial investigation determined the cause of the event was due to due a circumferential void located at the locking feature of the device's distal band. This void may have prevented the epoxy from locking the distal marker band into place which allowed the band to become detached during the procedure. However, the investigation continues and a supplemental mdr will be submitted after the final investigation has been completed, and will contain investigation findings and investigation conclusion codes.
