Event or Problem Description
On (b)(6) 2021, abbott point of care was contacted by a customer regarding act celite cartridges that yielded multiple act starouts on a patient in the cvor while performing a bypass. There was no patient information available at the time of this report.Customer reports that the procedure was successful and the patient is okay.Return product is available for investigation.(b)(6).The reporting decision was based on the limited information available that suggests the product was not performing within the variability of the assay. The customer states that there was no impact to patient management and the procedure was successful.Initial analysis showed that results were generated during the procedure on analyzers (b)(4).Analyzer (b)(4) did not yield a result and was only used for one test during the event.Currently there is no reason to suspect a malfunction exists. However, it was determined a potential delay has occurred.Analyzers (b)(4) were replaced at no charge to the customer and will be investigated separately. The investigation is underway.Reference art: art: 714374-000, instead of results stars appear in place of results if the analyzer detects that the sensor's signal is uncharacteristic.Since the sensor check is part of the i-stat quality system, an occasional result will be flagged due to a bad sensor.Other causes of this flag are improperly stored cartridges or an interfering substance in the patient's sample, either extrinsic, such as the wrong anticoagulant, or intrinsic such as medication. Also, aged samples may contain products of metabolism that can interfere with the tests.".
