It was reported that the procedure was performed to treat a lesion in the left circumflex artery using a.014 ht turntrac flex guide wire.As the guide wire was being removed, the radiopaque tip could still be seen on angiography, indicating a separation.A guide catheter was advanced to remove the separated portion and the guide catheter and guide wire were removed as a unit.Additionally, after removal the coils were noted to be elongated/stretched.There were no adverse patient sequela and no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
A visual inspection was performed on the returned guide wire and the reported stretched coils were confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints reported from this lot. There was no damage noted to the guide wire during the inspection prior to use which suggests a product quality issue with respect to manufacture, design or labeling did not contribute to the reported difficulties. The investigation determined the reported stretched coils appear to be related to operational circumstances of the procedure. In this case, it is likely that during retraction, the guide wire became caught on the anatomical and/or other devices used in the procedure resulting in the reported stretched coils.Since there were no separations reported by the account, it is likely that the noted separation and core bends likely occurred during packing for return analysis. The noted teflon coating damage likely occurred during retraction through the torque device and/or other accessory devices used in the procedure. There is no indication of a product quality issue with respect to manufacture, design, or labeling.B1: adverse event was removed b2: required intervention was removed.H1: type of reportable event changed to malfunction.H6: medical device problem code 1562 was removed.H6: health effect - impact code 4641 was removed.
