Medtronic received a report that the apb-1.5-2-hx-es coil was delivered to the aneurysm, but when the physician attempted detachment, it failed. The physician tried about 10 times with the instant detacher (id), an unspecified number of times with a second id, and it was unknown if a manual detachment attempt was made. However, it was reported the physician attempted by using hands. There was no issue reported with the instant detachers, and they were discarded.A replacement coil was used instead. In the same procedure, a apb-1.5-3-hx-es coil could not pass the microcatheter hub due to resistance in the sheath.A replacement coil was used for this as well. When the coil condition was checked, the coil was found stretched. It was indicated that all devices were prepared as per the instructions for use (ifu), and no patient symptoms or further complications were reported as a result of this event. The patient was undergoing surgery for treatment of an unspecified aneurysm, and unknown vessel tortuosity.Ancillary devices include an sl-10 microcatheter.
