Event or Problem Description
Medtronic received a report that the pipeline failed to open distally. The patient was undergoing treatment for an unruptured, saccular aneurysm located in the right ica.The max diameter was 14mm, and the neck diameter was 8mm.The patient's vessel tortuosity was severe. The landing zone was 3.12mm distal and 4.16mm proximal. dual antiplatelet treatment was administered, and the pru level was 80.It was reported that while attempting to deploy the pipeline, the distal end had the "flower effect." the doctor recaptured it and tried again and was able to get the distal end to open with the wire/catheter.The device was placed successfully, but with much manipulation to get it to open. The pipeline was not positioned in a bend, and less than 50% had been deployed when it failed to open. The pipeline had been resheathed 2 or less times.Full wall apposition was achieved, and angiographic results post procedure were said to be great. the patient did not experience any injury or complications. The devices were prepared according to the instructions for use (ifu).
