Block h6: medical device problem code a27 is being used to capture the reportable event of aborted/cancelled procedure.Block h10: the returned radial jaw 4 biopsy forceps was analyzed, and a visual evaluation noted that the clevis arm was returned cut and it was bent.No other issues with the device were noted. The reported event was confirmed.Based on all available information, it is possible that during attempts to remove the device an excessive force was applied to the device which results in clevis arms bent.Forcing the device against significant resistance could result in device damage or loss of functionality. Therefore, the most probable root cause is adverse event related to procedure.A review of the device history record (dhr) confirmed that the device met all material, assembly, and product specifications at the time of release to distribution.
