It was reported by (b)(6) that during service and evaluation, it was determined that the cutter device failed the visual inspection. It was determined that the cutter was stuck inside the attachment device and it was hard to insert the cutter into an attachment, but still possible to lock the assembly into a handpiece device. It was observed that the cutter showed discoloration, traces of foreign substance and corrosion on its shaft. The diameter was measured, and it was determined that it exceeded the tolerance of 0.01mm (millimeter) where the foreign substance and discoloration was detected. It was determined that the cutter blades showed regular signs of wear.It was noted in the service order that the cutter device was stuck in the attachment device and could not be released. The reporter stated that the cutter information was unable to be determined because it was stuck in the attachment. The devices were used together with the drill device. This event did not occur during surgery. There was no patient involvement. There were no reports of injuries, medical intervention or prolonged hospitalization. All available information has been disclosed. If additional information should become available, a supplemental medwatch will be submitted accordingly.
