Event or Problem Description

It was reported that the procedure was performed to treat an 85% stenosed, mildly tortuous, and mildly calcified lesion in the distal right coronary artery. The 3.0x20mm nc trek rx balloon dilatation catheter (bdc) was prepared per the instructions for use.The bdc was inflated to 12 atmospheres; however, it deflated and would no longer inflate. The balloon catheter was removed from the patient anatomy and once outside it was tested on the back table and a leak in the distal shaft was noted. The procedure was successfully completed with a 3.0x20mm nc trek bdc. There were no adverse patient effects and no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
Visual, functional, and scanning electron microscopy inspections/analysis were performed on the returned device. The reported leak and inflation issue were confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents and/or complaints from this lot.The investigation determined a potential product quality issue based on the reported leak, inflation issue and a noted longitudinal tear on the outer member proximal to the proximal seal. The issue is being addressed per internal operating procedures.Abbott vascular will continue to trend the performance of these devices.
