Event Description
|
Physician was attempting to use hawkone directional atherectomy along with non-medtronic 6fr sheath and 0.014" guidewire during procedure to treat a moderately calcified plaque lesion in the left distal anterior tibial artery (ata).The vessel is little tortuous with 3mm diameter.The vessel was not pre dilated but post dilated.Ifu was followed.It was reported that during initial advancement, the device was unable to cross the lesion.A nanocross pta balloon crossed the lesion.During withdrawal of hawkone, resistance was felt and the tip detached.The tip separated at hinge pin.Neither guidewire prolapse, tip damage nor embolization occurred.It was reported that as device was being brought back into the sheath for withdrawal, resistance was felt as it came around iliac bifurcation.As he pulled harder the entire cutting unit and nose cone separated from the catheter inside the sheath.Entire sheath was brought out of the body with broken hawk inside sheath.No pieces were left inside patient.Another sheath was inserted and case completed with balloons.There was no patient injury.
