Additional Manufacturer Narrative
H3, h6: the device, used in treatment, was not returned for evaluation but the images provided were reviewed, and the loosening was confirmed. The clinical/medical investigation concluded that, the information and imaging provided appear to support the failure of the cement/implant interface; however, the root cause of the reported event could not be fully assessed without the return of the explanted components. It is unknown if the type of bone cement applied could have contributed to the reported event, but the failure is more than likely related to the cement and not to the device itself. The assessed patient impact was the reported aseptic loosening/ failure at the cement/implant interface and subsequent revision tka.Further patient impact could not be assessed based on the information provided. No further medical assessment could be rendered at this time.Should additional clinical documentation become available in the future, the clinical/medical task may be re-evaluated.A complaint history review found related failures for the listed device; this failure mode will be monitored for future complaints for any necessary corrective actions.A review of the risk management file and instructions for use documents revealed this failure mode was previously identified. At this time, we have no reason to suspect that the product failed to meet any product specifications at the time of manufacture.Factors and/or some potential probable causes that could contribute to the reported event have been identified as abnormal motion over time, bone degeneration, fit/sizing, lack of ingrowth and/or traumatic injury.Based on this investigation, the need for corrective action is not indicated.Without the return of the actual product involved, our investigation could not proceed.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
