Additional Manufacturer Narrative
H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The following were reviewed as part of this investigation: patient severity, applicable previous investigation(s), sample analysis, applicable fmea documents, applicable manufacturing records, and labeling.Based on a review of this information, the following was concluded: the complaint of a breach in the catheter was confirmed, but the cause could not be determined from the photograph that was provided for investigation. The image showed a segment of powerpicc tubing in a clear plastic bag.The tubing was bent, which showed what appeared to be a semi-circumferential breach in the tubing breach in the tubing.Since the video demonstrated the reported event, the complaint was confirmed; however, the characteristics of the leak site could not be discerned from the photo since the damaged area was not in focus.Although the exact cause could not be determined, potential contributing factors include material fatigue and sharp instrument damage.H3 other text: device evaluation findings are in the manufacturer's note.
