Investigation: bd had not received samples or photos for investigation. To further investigate, retention samples from bd inventory were evaluated by visual examination for barrier gel characteristics and presence of additive, and upon completion, no issues were observed that would lead to hemolysis or erroneous results as all samples met specifications. The customer has acknowledged that phlebotomy technique was the cause of their reported issue.Bd recommends appropriate technique reviews to mitigate hemolysis occurrences.Based on a review of the device history record for the incident lot, all product specifications and requirements for lot release were met. There were no related quality issues during manufacturing of the product. On jun.29, 2021, a complaint history review was conducted and only the current complaint was found relating to the incident lot number and the ¿as reported¿ defect code.Based on the investigation performed, this complaint is unable to be confirmed for the indicated failure mode of hemolysis and erroneous results.Bd was not able to identify a root cause for the indicated failure mode.
