Event or Problem Description
Medtronic received information regarding a concerto coil that detached prematurely. It was reported that the coil was inserted and advanced through the microcatheter. Before reaching the target location, the coil detached from the wire. The wire was removed and the physician tried to use a coil pusher to collect the rest of the coil from the microcatheter. However, the pusher could not be advanced through the catheter and it was found that a portion of the coil was stuck distally in the microcatheter. The entire system was removed together; nothing was left in the patient. There was no harm or injury to the patient.
