Additional Manufacturer Narrative
Evaluation of the returned packing coil lp confirmed that the pusher assembly was kinked.Kinks typically occur due to forceful advancement against resistance.Based on the reported complaint, resistance was experienced, and the pusher assembly became kinked.Further evaluation revealed that the pusher assembly mid-joint was retracted proximal to the introducer sheath friction lock.If this occurs, resistance may be experienced during advancement.Forceful advancement against this resistance likely contributed to the kink observed near the pusher assembly mid-joint.During functional testing, the pusher assembly mid-joint was advanced from its returned position with resistance and could not be advanced any further due to the kink in the pusher assembly near the mid-joint.Further evaluation revealed additional kinks throughout the length of the pusher assembly. This damage was incidental to the reported complaint and likely occurred during packaging of the device for return.Penumbra coils are visually inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.

Event or Problem Description
The patient was undergoing a coil embolization procedure to treat a renal bleed using a packing coil lp, ruby coil lps, non-penumbra coil, and a non-penumbra microcatheter.During the procedure, the physician attempted to advanced two ruby coil lps into the target location; however, both ruby coil lps would not form and stay in place due to the high flow of the vessel. Therefore, both ruby coil lps were removed.Subsequently, the physician placed a non-penumbra coil and when advancing a packing coil lp through its introducer sheath, the physician experienced resistance and, the pusher assembly of the packing coil lp became kinked. Therefore, it was removed. The procedure was completed using a new packing coil lp.There was no report of an adverse effect to the patient.
