Patient information was unavailable from the site.A software analysis was initiated.Analysis found that the behavior was consistent with a known software anomaly in the medtronic navigation software anomaly tracking database.Concomitant medical products: product id: 9736226, serial/lot #: (b)(4).If information is provided in the future, a supplemental report will be issued.

Event Description
|
Medtronic received information regarding a navigation device being used outside of a procedure.It was reported that the navigation system displayed an error 64 message.Prior to the error message, it was noted that the navigation system was running slowly following a first patient registration.Following an additional registration, the error message appeared.It was noted that subsequent procedures for the day were completed without navigation from the outright.There was no patient present when this issue was identified.The reported issue was suspected to have been caused by corrupt patient exams.Additional information was received stating that there was a patient present when the reported issue occurred.It was unknown what type of procedure was being performed.The surgeon completed the procedure with the use of the navigation system.

Additional information was received stating that the manufacturer representative talked with the site and stated to follow the scan protocol but that they also need to keep an eye on this issue.Because this could be an issue with the software and it was recommended to upgrade the software.
