Additional Manufacturer Narrative
Upon receipt, the icd was interrogated, revealing the eri battery status. The icd was implanted for about 66 months and 22 charging cycles were recorded to the device memory. The amount of charge taken from the battery was verified, indicating the eri battery status to be normal and as expected. The memory content of the icd was analyzed. The analysis revealed that the icd was re-initialized on june 29, 2021.Therefore, the root cause of the reported safety backup mode activation could not be determined, based on the data available for analysis. Please note, the ability of the device to deliver therapies is not affected by the safety mechanism.Furthermore, based on the shock holter data, it cannot be excluded that a shock was delivered while the icd was in safety backup-mode.However, the root cause for the reported shock delivery was not conclusively determinable. Please note that, in general the backup mode program will be loaded from an unalterable memory. Therefore, the vf detection criteria are fixed in backup mode to cover the widest possible patient population. In a next step, the ability of the device to deliver therapies was verified. The anti-bradycardia pacing pulses proved to be normal and in amplitude and frequency as programmed.A fibrillation signal was applied and the device delivered a defibrillation shock as specified, documenting a normal and expected sensing and shock delivery. In particular, the specified energy level was reached.Further, a sensing test was performed and the device sensed the applied heart signals free of noise, proving the sensing function of the icd to be normal and as expected. There was no indication of a device malfunction. The manufacturing process for this device was re-investigated and all production steps were performed accordingly. There was no sign of any inconsistency during the manufacturing process.Particularly the final acceptance test proved the device functions to be as specified. In conclusion, there was no indication of a device malfunction.
