Event Description
Medtronic received information regarding a guidance system being used during a spinal procedure.It was reported that a screw placed during a l4-s1 fusion procedure on (b)(6) 2020 was found to be 1 cm inaccurate.The left l4 screw was deviated medially through the joint and had penetrated into the spinal canal.A revision surgery was done.

Event Description
Additional information received from a manufacturer representative reported that fluoro images were taken during the procedure and the surgeon confirmed accuracy.No problems were experienced during the initial procedure.The cause of the deviation was determined to be a medial skive potential and a drill bit taking the path of least resistance down the joint.The patient experienced foot drop after the procedure.
