Medtronic received information regarding a marksman microcatheter and pipeline device that had resistance during delivery. The patient was undergoing a procedure for coil embolization and flow diverter implantation via femoral approach to treat an unruptured amorphous aneurysm of the right cavernous sinus segment. The aneurysm max diameter was 12mmand the neck diameter was 6.3mm.Vessel tortuosity was moderate. It was reported that the marksman microcatheter was in place. The pipeline distal end failed to open when less than 50% deployed and the pipeline could not be pushed out the distal end of the catheter. No other devices or steps were taken to open the pipeline. The devices were removed from the patient. It was noted all devices were prepared and the catheter flushed as indicated in the instructions for use (ifu). The procedure was completed with spring coil embolization and implantation of an "ordinary stent." there was no harm or injury to the patient.
