Event or Problem Description
This is being filed to report that the clip detached from one leaflet, recurrent mitral regurgitation (mr) and leaflet injury. It was reported that the initial mitraclip procedure was performed on (b)(6) 2017 to treat unk mitral regurgitation (mr) with a grade of 4+.One clip was implanted, reducing the mr to 3+.On an unknown date, a control echocardiogram was performed, which confirmed that the clip detached from the posterior leaflet and remained attached to the anterior leaflet (single leaflet device attachment/slda) and mr increased to 4+.It appears there may be some leaflet damage from the slda to the posterior leaflet. There is a p2 flail leaflet and flail chord but it is not certain if this was preexisting prior to the initial clip placement. The patient has been increasingly symptomatic in the last 6 months. The slda is suspected to have occurred 6-12 months after implantation according the physician.A second mitraclip procedure will be performed at a later date.No additional information was provided.

Additional Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar incidents reported from this lot.Based on available information, a cause for the reported single leaflet device attachment (slda) and tissue damage could not be determined. The reported mitral regurgitation (mr) was a cascading event of reported slda. The reported medical intervention and hospitalization was a result of case-specific circumstances. The reported patient effects of tissue injury and mr are listed in the instruction for use (ifu) as known possible complication associated with mitraclip procedures. There is no indication of a product issue with respect to manufacture, design, or labeling.
