H.6.Investigation: bd had not received samples, but four (4) photos were provided for investigation.Three (3) photos were reviewed and the indicated failure mode for broken tubes/damaged case was observed. One (1) photo was reviewed and foreign matter (fm) was not observed as the picture showed normal additive pellets from the freeze-dried process.Additionally, one hundred (100) retention samples from bd inventory were evaluated by visual examination and the issues of broken tubes and fm were not observed.Based on a review of the device history record for the incident lot, all product specifications and requirements for lot release were met. There were no related quality issues during manufacturing of the product. This complaint has been confirmed for the indicated failure mode of broken tubes. However, the complaint could not be confirmed for fm as the tubes met specifications. The exact cause for the broken tubes and damaged case was unable to be determined.
